New Employee

1. Update LikhaInternet Team Directory

2. Open BPI Payroll Account

Requirements:

- 2 Valid ID’s (Company I.D and other I.D), Barangay Clearance, Cedula, Proof of billing (water bill or electric bill), TIN number, Authorization Letter to BPI for Creation of Payroll Account

3. Encode 201 in google sheet

4. Add Employee to the Salary Structure

Add New Employee to Contribution List at SSS, HDMF and PhilHealth

How to Add New Employee (PhilHealth)

1. If No phic number > submit photocopy of birthcert, Valid Id and Company Id > Fill up PMRF Form https://www.philhealth.gov.ph/downloads/membership/pmrf_012020.pdf

2. If with phic number > photocopy of valid id only > Fill up PMRF Form https://www.philhealth.gov.ph/downloads/membership/pmrf_012020.pdf

3. Submit ER2 Form : https://www.philhealth.gov.ph/downloads/employer/er2.pdf

How to Add New Employee (SSS)

1. If no SSS Number > 2 Valid ID’s (Company I.D and other I.D) > go to > https://member.sss.gov.ph/members/rcsmi/main.html

2. With SSS Number > Log in sss.gov.ph

3. Click Payment Reference Number ( PRN)

4. Click Cancel > Reason ( Add Employee)

5. E-Services Tab > Submit Employment Report R1A > Fill up data then submit to declare new employee

How to Add New Employee (Pagibig)

1. If not yet a pag ibig member > 2 Valid ID’s (Company I.D and other I.D) > go to > https://www.pagibigfundservices.com/PubReg/NewReg_Page.aspx?88D64A1E7CDAFB6882DDFEB76443D811

NOTE: Your employee needs to be a member of SSS before he/she can register in HDMF.

2. If already a pagibig member > Go to Documents - Admin Folder

3. Click Government Bills - Copy

4. Open Pag Ibig File Generator

5. Add Employee with his/her MID Number

2. Maternity Notification (thru online)

3. Submit MAT-2 >https://www.sss.gov.ph/sss/DownloadContent?fileName=SSSForms_Maternity_Reimbursement.pdf

4. Fill out the form

6. Total Monthly Salary Credit

7. Daily Maternity Allowance (multiply by ) Number of Days
